This Buckle Up for Life user account is free for me to use in my capacity as a team member or representative of a partner organization registered with Buckle Up for Life.

I understand that if I am the first person to submit a Partner Registration for my organization, using our unique EIN/TIN (or equivalent for non-US partners), then I will immediately receive full access to my organization's account. I understand that if an active Partner Registration already exists for my organization, at the time my user request is submitted, then my individual team member access will remain limited until another, pre-existing member of my team approves my request. I also understand that once I am granted full team member access, I will be trusted to help manage (approve, deny, or terminate) access for additional/subsequent members of my team.

I am responsible for maintaining and updating the information associated with my user profile (including, but not limited to certifications, expiration dates, role status, language fluency, Etc.).

I understand that the information I provide within my user profile (including, but not limited to certifications, expiration dates, role status, language fluency, Etc.) will be reviewed alongside any subsequent partnership application(s) submitted by my organization, and can impact the strength of their assessment, accordingly.

I will not share any Protected Health Information (PHI) regarding families or individuals I serve through my program with Buckle Up for Life. No identifying information should be provided, unless specifically requested, and unless the appropriate consent/releases have been voluntarily completed and provided by relevant parties.
